Maine-Dartmouth Family Medicine Residency (MDFMR) offers a Plus One residency in Neuromusculoskeletal Medicine and Osteopathic Medicine to provide education and certification in the integration of advanced Osteopathic Manipulative Medicine into primary care. Significant demand from patients and residents prompted the creation of this fellowship. MDFMR has a history of integrating osteopathic principles and practices into patient care since 1996 and adding a Plus One Year is consistent with its mission to provide holistic primary care.
Upon graduating from a Plus One program residents would be expected to provide OMM in full spectrum patient centered primary care including inpatient and outpatient medicine, with a broad based knowledge in the community, family and socioeconomic aspects of medicine. During their year, Plus One residents would be integral to the provision of care and teaching in the family medicine residency and UNECOM Core Training Center at MDFMR.
We are also looking forward to interdigitating the Plus One curriculum with a special emphasis curriculum on integrative medicine that is being implemented at MDFMR.
The curriculum offers training in a variety of settings.
Neuromusculoskeletal Medicine Fellowship Curriculum
General
The program will comply with the Basic Standards for Plus One Residency Training in NMM/OMM. The basic structure will consist of
...
- 4 ½ day sessions outpatient clinic, at least 3 of which will be continuity
- Daily inpatient rounds and OMM consults supervised by faculty with NOBNMM certification
- Weekly “Neuromuscular Assessment Team” meetings. This will be a multidisciplinary team including NMM, psychiatry, psychology, social services, rheumatology, physiatry, integrative medicine and other specialties on a rotating basis to review cases and develop a comprehensive plan of care
- Weekly educational sessions during which time Plus One residents will receive one on one didactic education and provide teaching for students and residents
- Weekly sessions precepting family medicine residents in continuity care including OMM
Continuity of Care Clinic
MDFMR has two continuity outpatient centers in family medicine, which also employ pediatrics, OB/Gyn, dermatology, psychology and social services. There is significant demand for OMM services by non-osteopathic providers. The Plus One Residents’ 150 patient continuity practice and consult service will be drawn largely from this population.
Hospital Care
MDFMR is closely affiliated with MaineGeneral Medical Center: two campuses of community hospitals of approximately 150 beds each plus the Alfond Cancer Center. There is a long history of osteopathic medicine at these sites and OMT is already a routine part of credentialing. Plus One residents will participate in a consult service supervised by NBONMM certified providers and assist in teaching osteopathic family medicine residents OMT in the inpatient setting.
Academic Training
MDFMR has an extensive didactic curriculum consisting of daily noon lectures, weekly afternoon long educational sessions and daily morning rounds. There are also weekly didactics for medical students containing lecture and patient-based OMT workshop components. Plus One residents will be expected to participate in these sessions both as learner and teacher. This will provide them with experience teaching at the resident and student level both in lecture and tutorial format. Plus One residents will also have didactic time of their own to review advanced OMT, documentation and teaching principles. They will also use this time for scholarly activity supervised by faculty including PhD educators. As part of the practice management curriculum, Plus One residents will be expected to participate in the weekly faculty meetings that pertain to issues around the running of the continuity centers.
The Northeast Osteopathic Medical Education Network (NEOMEN), our OPTI also provides daylong educational sessions in OMM, which occur twice a year and include residencies from Maine and New Hampshire. Plus One Residents will be expected to participate in these sessions as educators.
OMM
Plus One residents will be expected to become competent in all techniques of OMM and their indications and contraindications as related to the adult, pediatric and geriatric population. There will be particular emphasis on OMM as it relates to the management of chronic disease in primary care. Plus One residents will also become facile as consultants in NMM. This will be achieved through participation in patient care, the Neuromusculoskeletal Assessment Team and time with physiatry, anesthesiology and physical therapy.
Behavioral Science
MDFMR employs a behavioral scientist who will participate in the Neuromusculoskeletal Assessment Team. There will also be reading assignments on psychology as it pertains to chronic disease management, patient participation in care, and the role of psychological trauma in pain. Plus One residents will also participate in the residency behavior curriculum as part of the noon lecture series.
Geriatrics
MDFMR is privileged to have a fellowship in geriatrics headed by Karen Gershman, MD which attracts a large geriatric population to the practice of MDFMR. Plus One residents can participate as consultants in geriatric assessments. Maine is also a state with a high percentage of elderly in the general population so geriatric medicine is inherent in all aspects of medical care.
Internal Medicine & Surgery
Plus One residents will experience internal medicine and surgery as an integral part of their continuity and inpatient experiences. They may also get experience in OMM as it pertains to urgent and emergent care by participating on call.
Pediatrics
Plus One residents will gain experience in pediatrics primarily through providing OMM to newborns on the MDFMR service and as teaching osteopathic residents to perform structural exams & treatment as part of well childcare in the continuity clinic, and the OMM consult service. MDFMR is also participating in a grant to provide pediatric care to participants in an area homeless shelter. The model for this project is to provide multidisciplinary assessment and treatment, which may include osteopathic assessment and will provide an example of patient centered longitudinal care.
OB/Gyn
MDFMR has a busy obstetrical practice and is embarking on group visits for prenatal care. Plus One residents will provide and teach OMM on family medicine residents’ continuity patients. MDFMR’s inpatient service includes obstetrical patients so there will be an easy venue for exposure to OMM in the immediate post partum period and potentially intra partum period as well.
Integrative Medicine
MDFMR has a long-standing history of incorporating broad thinking in the care of its patients and has faculty with expertise in herbal medicine, acupuncture and nutrition. We are excited to be embarking upon an online curriculum in integrative medicine with the University of Arizona, which will be a core part of the family medicine and neuromusculoskeletal curricula.
Resident Evaluation/Core Competency
Plus One residents will be evaluated through portfolios, which will be based on the seven core competencies put forward by the American Osteopathic Association, and tied to the goals and objectives provided by the American Association of Osteopathy.

Kathryn Brandt, D.O., Kathryn served as the Director of Osteopathic Medical Education from 2002-2008. She has been the Director of the Neuromuscular Medicine Fellowship since 2009.
